They can *see* the unruly children, but *choose* to ignore them. That's

I call shenanigans on providers not seeing their unruly users. They
have no problems with bandwidth caps, or doing the dirty work of
copyright police, both of which require some level of network monitoring
per customer.
Keep in mind, most of the folks on this list are not representative of
the average consumer in terms of the skill-sets which are relevant in
this problem space.
I'm very well aware of that. I've got a fairly decent sized user base
that I consult for, including small biz and end users, so I'm painfully
aware of how... less then spectacular consumers are when it comes to
even the most basic computer tasks.
