I don't know about that. I don't use MH format folders... I use
Maildir myself, and I've never had any problems with getting the N-flag
right. For mbox-folders, people have reported there being problems too,
although mostly those would be non-Mutt related.

Mutt won't show a counter, although you're right it should be simple and
also fast to do in the implementation. The boolean flag works right for
me.

In theory, you should be careful to make sure you create unique
filenames. The Maildir spec also specifies that new messages should
first be created in $box/tmp, then linked/moved to $box/new.
There's programs (such as maildircat or something like that) which just
take a message at STDIN and dump it into the specified Maildir folder.
These could be useful if your MDA doesn't do Maildir format natively.
Searching the qmail home page (http://www.qmail.org) should provide
some pointers...
