I did try a couple of extra things (install 4.2.5, uninstall Android SDK and re-install manually outside of %localappsettings%) and I also ran VisualStudio in Admin mode a couple of times and rebooted my computer, but for the most part I didn't do anything. I just noticed a couple of days later Mono Android was working now. I can't explain the change but maybe just be patient?
I'm surprised that that Android Designer doesn't show up in the "Open With" menu.
