Maybe you can also add a link to the mjpegtools where they find the
sources of the AVI reader/writer used in the mjpegtools. 

As they mentioned there are at least 2 differnet version of mjpeg
encoded AVI's aviable. And than they can be extended using the Open DML
(if I remember correct) spec to work around the 2GB limit on 32Bit
architectures. 

I also checked their homepage, and I found nothing more concrete that
they support "different formats" or so.
