I'm glad to see I'm not the only one that has seen this happen.  I've been trying to 
find another clip that has an obvious enough spot where this happens with an R value 
of 0 but haven't found a scene that looks nearly as bad as the clip I initially saw it 
on - and I'd removed the pre-mpeg2enc file :-(  I did find 1 scene that seems to 
exhibit it just a little bit.  It'll take me a while to grab just a small part 
though...it is about an hour into a 2 hour clip, recovered in nuv format...which of 
course doesn't have navigation headers :-)

It most resembles trying to encode something with a lower bitrate - as if using -R 0 
is messing with the bitrate calculations or something.  Although the scene I had 
didn't have much motion at all, it did have a lot of detail in the scene itself.

Anyway...still trying to put together a short clip that produces the problem for 
Steven.
