I can see how just using the last 4 digits of a social along with initials 
could result in dups.  It might be far-fetched, but suppose that mine is JC1234 
(111-22-1234).  There could be a Julie Carson who just happened to have a 
social with the same last 4 digits (and different beginning ones 222-11-1234) 
who worked at my hospital.  Or. . . are there enough numbers in the SS# system 
that the last 4 digits are NEVER duplicated?  

 

In the local grocery store one day, I ran into a woman who had the SS# 
immediately preceding mine.  My (then-) husband asked, “Did she look familiar?”

If you want something that will never change use the first and middle initial 
(or N if no middle initial) and the last 4 numbers of the social security 
number or the employee number if you have one. 

 

This way there should never be a duplicate.

We are a new MEDITECH site and are having the discussion on how to set up 
mnemonics.  How have other hospitals done it?  We want to assign one mnemonic 
that does not change irregardless of name change, marriage or divorce.  We 
basically do not want inactive users in the database.  Any thoughts or lessons 
learned.  We would like to just to first initial and last name, but know that 
with divorces that could get sticky if we do not change the mnemonic for sign
