Hi, I've been having these problems too. But to me there seems to be too separate, though perhaps connected, problems.
First is a problem I only have at the login screen. This problem occurs even though I'm not using a bluetooth headset. On awaking from sleep or prolongged inactivity VoiceOver will not talk. I will typically still get system thunking/beeping sounds, though not always. If I just type my password and press return, I'm logged into my account and VoiceOver starts working again. Alternatively, if I click the trackpad (anywhere) VoiceOver seems to focus on the login screen again and I can do things as usual. This problem doesn't always occur, but seems to be more often than not. It happens both on my mid 2011 iMac and my mid 2011 MacBook Air, both running Mountain Lion. I don't think this ever happened with Lion. The second issue is specifically with Bluetooth headsets. It's only happened to me a few times, because I've been too frightened to try bluetooth headsets ever since. It happened first under Lion and second under Mountain Lion. After my bluetooth headset had turned off or ran out of batteries, I would lose VoiceOver. On charging/turning on my bluetooth headset, I would get system sounds again but not VoiceOver. Once under Lion this even happened immediately after I switched to the bluetooth headset. This would leave me in the position of needing to force a restart or get sighted help, both of which I like to avoid. I've only ever tried bluetooth headsets on my iMac.
