Hi, As Ricardo mentioned, iTunes will usually give the busy message when first accessing the iTunes Store. Have you subscribed to iTunes Match or do you have an iDevice wirelessly syncing with iTunes? iTunes Match will cause iTunes to access the Store right away when iTunes starts but normally is ready within seconds. If your iPhone or iPad syncs wirelessly to iTunes, it will do that upon startup of iTunes thus will make iTunes seem sluggish in the beginning. Also, as Ricardo pointed out, some views in iTunes will make VO sound the busy message but I've never had it have problems when in List view. I've also had iTunes/VO get upset when I'm impatient, that is, if I start trying to navigate with VO too quickly as things are being setup within the app.
So, when starting iTunes, it is desirable to wait until you hear VO announce the active app and where focus has been placed. If possible, navigate left to the LCD section and see if iTunes will tell you what process is happening. I realize that this may be impossible if the busy message is going on but sometimes you can move between busy signals. Let me know if anything improves. Later...
