Stupid question perhaps, but are you holding the Nano so the wheel is at the bottom? Did you try resetting it from iTunes? I have not experienced this problem with my Nano, but I have on at least one occasion found it necessary to reset the Nano completely, which meant I had to basically start over, but it had gotten really hosed in some fashion.
