Questions for you.

1. Which version of LyX are you using? LyX 1.3.4 allows you to use a
textclass fro which the LaTEX files are not installed. (Obviously,
you can't actually typeset your document, but you can write the
text.)

2. Can LaTeX find your installed nih.cls file? Ie, does a simple .tex
document that uses it (as described in the docs on the dartmouth web
site) compile.

Nothing to do with lyx. Maybe created by whoever packaged your version
of LyX.

If you run lyx from the command line and Edit->Reconfigure, you should
see a whole heap of information. Does any of this help?
