I've glanced through it. There's examples starting on p17. I'd suggest you grab the text from one and try to format it in LyX, asking specific questions as you go. The first would be how to replicate those odd line numbers, as they look to be the most difficult (to me, anyways, perhaps they're a solved problem to others). The numberings, indentation, etc all looks pretty straightforward. Best of luck.
