I recommend you prepare a lyx file using standard lyx editing techniques (i.e. document and paragraph layouts) which is as close to what you want as you can get it. Then email the list for help on specific formatting requirements you were not able to accomodate. The document you referenced is 121 pages long, so I doubt that anyone here will take a second look at it.
