This one is known, and we do not have yet a solution. A workaround is to increase then decrease the zoom level. The previews will be regenerated.
(4) The positioning of display equations in the Lyx interface is to the left. The equation label font is very big and pushes the equations to the left.
Can you provide a screenshot (not too large, we limit attachment size) to show the issue?
