I really don't like focus following mouse and as soon as it was possible to shut it of on our X-Terminals and then Linux workstations (changing from twm to fvwm) I did it. I like to not pay attention to where the mouse is and do a lot with the cursor. I don't know if it is possible to create windows which never get input focus, but I think it should be. And this is definitively the way to go for dialogs with only buttons and no "keyboard" input fields.
