LyX's shortcut notation is inconsistent

While updating the docs to follow the current shortcut notation, I noticed that 
it is not correct:

We use "Alt+D". This implies to use "Alt+Shift+d". To be consistent, we should 
use either
"Alt+d"
or
"ALT+D"

Any thoughts?
