The problem exists regardless of whether the paragraph within which the
inset is embedded is RTL or not (but it's much harder to see what's
going on if the surrounding text is not RTL, because then the inset text
overwrites the text which is to the left of the inset...!)

I'll have a look. I've tried hard to not break RTL during my cleanup but
I obviously introduced some new bugs :-(
