What part are you finding difficult?  What have you tried?

Aligning the lyrics to the music. Just formatting the music is easy.

Splitting both tune and lyrics into chunks and aligning each chunk separately quickly becomes tedious.


To make two voices, use << {  } \\ {  } >>
To get a smaller notehead, try \magnifyMusic #2/3 { "note here" }
To keep the smaller notehead from merging with the larger, I'd try putting a very short spacer grace note in front of the smaller note, like \grace { s1024 }
