Check class of service for the VOICE MAIL port.  Since the voice mail is
transferring the call, it must also have permissions to access CO lines and
complete a transfer.  (from experience)!

Are you thinking of "Transfer to CO" option also under "Class of Service"?
Yes this too is enabled and it seems to work fine. I called his extension
from a cell phone (with FWD disabled), answered it and transferred it to
another cell phone just fine.
