What you are asking is really no big deal. as long as you can get TWO clean pairs and are not looking for hi speed. There are any number of products that will work with no problem at 5 KM @ 19.2 over 2 pr of 24 ga wire.
As SMDR data is not "fast" data we have run SMDR data nearly 20000' from a CO to a customer's LSU ( local storage unit ) NEARLY 20 YEARS AGO using RS-232 Serial Extenders.
A Google search for RS-232 Serial Extenders should yield several hundred hits
