This sounds exactly like what I've been running.  It's part of the
"homebuilder's special" tailwheel setup that Wicks and Aircraft Spruce sell.
AS&S part number is 06-03500 $21.80 (see the bottom of
http://www.aircraftspruce.com/catalog/lgpages/homebuilder_tailwheel.php ).
I have over 800 "landings" on mine (triple that if you must count the
bounces), and it still has a lot of life left in it.  I bought a new one a
while back and when I went to install it, it looked just like the one on the
plane, so I set it on the bench waiting for the 2000 landing mark to arrive.
When you measure the thread's it'll look like 1/2", but there's a shoulder
on the axle which makes the 5/8" bearing fit perfectly.  Both of these
numbers could be off an eighth or so though.  I'll try to measure mine
tomorrow and let you know, but it's at the airport at the moment...
