While a fully automated bug reporting system may be a great objective, there may be ways to get most of the way there at a lot less cost - for example I like the way Postfix gathers bugs and leverages diagnostics with a specific trouble report command [postconf -n] that, in an organized structure, first captures the state of Postfix as it is deployed locally, and second Postfix can generate very detailed logs. When something goes wrong, the diagnostic effort gets leverage from these two areas and works rather well.
