-dpaper=letter tells troff to format for letter, but it doesn't pass that 
on to grops, which is still apparently selecting A4 as default; for 
correct formatting, you should specify *both* -dpaper=letter *and*
-P-pletter, so that both troff and grops know you want letter size output.
