Another difference is the kernel version Gentoo 2.6.11, Knoppix 2.4.23.

I've just tried Knoppix 3.8.1 which comes with Linux kernel 2.6.11, and
surprise, when tweaking the hd params with the same values (except geometry)
as before

/dev/hda:
 multcount    = 16 (on)
 IO_support   =  1 (32-bit)
 unmaskirq    =  1 (on)
 using_dma    =  1 (on)
 keepsettings =  0 (off)
 readonly     =  0 (off)
 readahead    =  8 (on)
 geometry     = 65535/16/63, sectors = 78198750, start = 0

the buffered read performance is 17,44 MB/sec as with Gentoo. What does this
mean?
