In addition to the paperwork (which needs checking in copyright.list by 
someone with access to fencepost.gnu.org), someone getting 
write-after-approval access should be "known to submit good patches".  
They may well be patches for a development branch rather than trunk, but 
there should be some patch submissions first, then write-after-approval 
access once a few have got to the point of being committed by someone 
else.
