Nat is a problem for any p2p, including TCP. Things like STUN, and the more advanced Nat Traversal protocols like H.460(18,19,23,24) still cannot work if they're trying to penetrate anything other than a braindead Nat device. None of these can traverse an intelligent nat device such as cisco ASA, or netscreen, or sonicwall, etc. Things like SIP, H.460, XMPP, and whatever Skype uses ...
