That old, horrible template was replaced immediately because it was universally hated. Not one ever filled out (I think Masayuki checked a couple of boxes once). Otherwise, no one. The current template is doing. Some people who should know better are ignoring it but a significant number of people are doing their best to write something in each space.But I think it would help to stop call then by judgmental names.
