Just to add some more information,
There are two ways of generating PO files,
The normal way, in which the comment is just a comment. if a term is
used several time in a piece of code, it is placed only once in the PO
file, and used as many times as needed in the code.
If the language needs to translate the same English term or sentence in
different ways for different parts of the program (such as when the
language has declinations), then a different set of PO files needs to be
generated, which differences each single use of a term or sentence, so
that you can translate in a different way in each case. To make the
strings different and identifiable, the comment that is normally written
outside of the msgid is included inside the msgid (but does not need to
be translated). PO files do not allow two messages with the same msgid.
The problem of using these is that you have many more translations to
make (about 10.000 more messages, I seem to remember, even if they are
translations of messages that you have already made before. Translation
memory does not work either, as these msgids are different. The messages
below belong to this type of files.
If you do not need to make different translations for the same term, it
is better to use the first type of files (which I believe are now
generated by default by po2oo in version 0.8 of the Translate Toolkit).

The first part og msgid is unique identifier, because these strings
(Dialog) are repeating.
You dont need to translate this, translation tools like KBabel do not
copy them into msgstr.
They are needed for improving translation quality in non-english
languages. EG. string 'Edit' can be translated in 3 or 4 different
ways in many languages, depending is this a verb or nomen or something
else :).
