There's still some rough edges to ClamAV, but it is definitely a very capable competitor to the commercial scanners. As regards the support of email scanning, prior to version 0.70, scanning email sometimes caused clamscan to fail, and scanning over clamdscan was pretty flaky. From version 0.70 and up, it seems to be pretty reliable. My servers scan over 10,000 emails a day, and it's done quite well.
