I had the exact same problems myself, with RH 8.0 and Sendmail, using clamav-milter. I asked a few times about what a solution would be to this, and never got much of a response. It just seems that clamav-milter is not ready for a production server and still needs some work, I guess.
I'm not sure of the status for clamav-milter, but IIRC it's clamd dying in most cases, due to problems in the mbox.c code. ALl my clamd problems went away after I disabled the "ScanMail" option in clamav.conf.
There _are_ big improvements to the mbox.c code in the latest snapshot/in CVS, but not much people seem to use it. Perhaps it's time to get a "better,but certainly not bug-free" release out, to get better feedback on the _current_ mbox code.
For me, amavisd-new + clamd + spamassassin is running like a charm; in the next days, I'll set up two load-balanced machines using this setup for a local ISP :-)
