We use the Formulator from Formulatrix - which is very good, and the software (Rockmaker) for designing screens is great and allows you a great deal of flexibility BUT it does sort of require you to buy into the whole Formulatix ecosystem to get maximum benefit, which may or may not work for you, depending on the size of your userbase.
