All but one or two of the blue entries for the chapter headings in the
ToC have one or more dots missing.
On page 140, the TimesNewRomanPSMT font was used, possibly in error.
On page 192, I suppose the text could be changed in another edition
(unless it's added now) to state that custom font sizes, such as 11.5pt,
other than those listed as being available could be directly entered in
the text box to be used for paragraph or character styles, in addition
to using percentage font sizes.

Thank you! When fixing those, I found many more bad x-refs in the
same part of the book and a few in other places. I hope I have
now found and fixed all of them.
