It seems that "H/W" corresponds to the left stereo channel and "H/W 1"
to the right one (I only have two speakers): I need to set these two
channels to the value "PCM out" to hear the output. Is this expected?
".

Yes and no.

The "yes" part is: if you want to hear PCM stream from a digital
source (.wav, .ogg, .flac, .mp3 file through some kind of player).

The "no" part is: if you want to hear sound from mic/line in - for
these on 7.1 you have to follow the instructions I gave.

I can't advise on 5.1 since I don't have the card.

Visit my http://appsfromscratch.berlios.de/ open source project.

Just to make things (hopefully) more clear:

"H/W", "H/W 1" are indeed left and right channels, for both
PCM playback and line-in.

Microphone input is mono, it is digitized by both "H/W", "H/W 1" if
you set them to be "H/W In 0", "H/W In 1".

The card by itself does not have HW mixing, so there is no simple
way (though it might be possible through 'dmix', I haven't tried)
to simultaneously hear line in and PCM out in the same "H/W" or "H/W 1"
channel.

For example, when I use Skype, I set, say

"H/W" -> "H/W In 0" (left),
"H/W 1" -> "PCM Out" (right),

so in my left ear I hear myself from my microphone and in my right ear I hear
the person I'm talking to as PCM Out.
