I didn’t run into the notarized thing at all.  Did you do the Sam.gov stuff 
before you applied for any of the rest of the EBB stuff?  The instructions said 
to do that step first so I did that before submitting the EBB thing.  After the 
EBB thing was approved I had to do the USAC part.  The USAC part was the 
easiest overall because when I sent in the wrong stuff she was REALLY good 
about responding with questions and then telling me what she actually needed.  
After the USAC part came back I got an email from Sam.gov saying that my email 
address needed to go in to accept an invitation for a role which I just did 
this morning and I was never asked for anything else.   I vaguely remember 
something about Third Party stuff but didn’t choose any of that so maybe you 
need to find that and clear it?

Where did you go to create your SAM account.  When I try to create one under 
SAM.GOV it tells me I need to used the new login.gov.  I created a login.gov 
account, but can't seem to figure out how to create a SAM.GOV account from that 
LOGIN.GOV

USAC  wants "Evidence that the provider had the same service offerings
