I didn’t end up there.
It requested the designated entity administrator which I set that to be myself, printed out the letter, had it notarized and sent it in.

Has anyone got as far as the SAM.GOV sign up for the EBB. I'm almost done with the sign up and it wants me to submit a notarized letter that it generated. They want to to use this random third party agent (see below). Looks a bit odd to me. I'm thinking about removing that paragraph before sending it in.

For the purpose of registering with the United States Government through the online System for Award

USAC wants "Evidence that the provider had the same service offerings
available on December 1, 2020". I'm not sure what type of evidence I
can give them. Pointer to website wouldn't help. I don't think we
have ever filed any FCC or USAC paperwork that shows our pricing. Was
thinking about printing out a December invoice from random customers
for each plan and blanking out the customer info.
