Option file and optionset

Can anyone give me a clarification on how include and excludes are organised
when some statements are specified in the option file of the client itselfand
some others on the option set defined on the server and associated to the
client. Is there a possibility to specify include or excludes that will be
inserted in the middle of the optionfile. Does the sequence specify the overall
sequence of all the inclexcl statements and will the be read also from bottom to
top?
