I'm sorry but after looking at the screenshot you sent us (seems to be PDF reader having left pane containing a TOC), I fail to see what's wrong in:

The "PDF TOC" in the screenshot contains:

Chapter 3. Secure world
    Section 3.1. AM4384 ROM
    Section 3.2. Primary protected application (PPA)
    Section 3.3. PA loader
---

This means that "* Chapter 3, Section 3.2 Primary protected application (PPA)" corresponds *exactly* to what the users sees in the TOC.

May be "Chapter 3, Section 3.2" is a bit redundant[*], but certainly not misleading.

The only way to get the result you want is to customize the XSLT stylesheets which are part of XMLmind DITA Converter, our open source, in-house, DITA processor. What to do and how to do it is out of the scope of normal support.
