Not always. As I said from my original post, I monitored the
installation using ProcMon (the successor to RegMon), and it looked like
everything was okay. The problem turned out to be per-user vs.
per-machine install. HKEY_CLASSES_ROOT isn't for the entire machine
unless you specify ALLUSERS=1, and ProcMon was masking that problem as
all of my registry calls looked like they were correctly going to HKCR.
As sports columnist Bill Simmons would say, "The lesson, as always, I'm
an idiot."

I've got a couple of more questions, but I'll ask them under separate
messages.
