Hmm ... if one is gonna be anal about this, I think "lost" should be used instead. Some users never really know their passwords or usernames, they are either recorded somewhere or recorded automatically (autofill?), and in case they loose this, then they never really forgot since they never remembered it in the first place. Either way, they're still lost.
