I don't know how things are working right now but I believe it would be great that when I patch is applied, the person who did the patch would also submit a description of what have been done. It could be like sending a message on the mailing list with a specific nomenclature like [web2py][mercurial id]Title of the change. This way everyone will see the changes and it will keep a record.
