This is a very intriguing situation and problem. I have taken 'online' exams before, and you're right, it is very tempting to switch to another application to look up the answers. It seems the most straightforward way to eliminate that -- and also to prevent the user from consulting their own written notes -- is to enforce a timeout on each test question. If you know the answer you can provide it within 15 seconds; if it takes longer, you're looking it up. Obviously that only applies to multiple-choice, not essay questions.
One other thought: I have seen applications produce a dialog box that MUST be answered, and the user cannot switch focus away from the application. Perhaps you could hack vncviewer to implement the same capability.
