You need to forward the port number that the VNC server is listening at for http connections (typically 5800) and port 5900. The Real VNC documentation has the required details for setting this up in the "Connections" tab of your VNC server and a link to follow for instructions on setting up port forwarding at your router.
Finally, if you are running a software firewall at the VNC server PC you will need to also open up the relevant ports for incoming connections. HTH.
