In my perfect world , to solve the problem with VO interrupting an important
call, would be for me to be able to preset different short tones for messages,
mail, incoming calls etc and VO and all of my alert not need to be altered to
quiet VO during a call. With the different tones I could decide answering the
alert is of immediate importance. And if I should need VO during a call all I
would need to do is remove the phone from my ear and touch the screen. I dont
mean having to use any specific gesture to turn VO. on or off while in a call.

Just my 0.02 cents.
