I've been facing the same issue for a long time. I wonder if others have a solution. In the meantime, here is my workaround.
When you delete a message, swipe right already without waiting for VO to re-focus on the next message. That way my focus has always been up to date.
I also delete a hundred or so messages a day, but it could also be that this doesn't work for you either because the number of emails coming in is more than mine. Let us know if that's the case, because then only someone with the same volume of incoming mail could experiment and help.

I will start by saying I’ve talked with Apple Accessibility, Apple Care, and Apple Support on this issue without success in achieving my goal. I use the native Mail app to read and delete between 100 to 300 emails a day. When I’m using the native Mail app, I want it to refresh and provide me with my current Inbox messages when I first open it but I don’t want it to refresh again while I’m using the app. I can’t tell you how many times I’ve been deleting an email that a refresh occurs causing the message with focus to change and my deleting the wrong message as a result. I’ve been into the Mail app settings for my Gmail account and have been unable to figure out what combination of settings achieves what I want. Turning off Push doesn’t do it and setting fetch to manual doesn’t do it. I’m using an iPhone 13 Mini running iOS 18.1.1 and would welcome any suggestions.

 

I would encourage you before making a setting suggestion to make sure yours is set the same and go into the native Mail app for a bit to ensure new messages aren’t arriving with the setting.
