IMHO, all audio, including voiceOver should go through a bluetooth speaker or head phones by default. If you don't want VoiceOver to go through an external speaker then use Air Play or Chrome Cast. Of course, if Apple could add a feature where you could send media to one bluetooth device while sending VoiceOver somewhere else then that would be a nice feature.

true, and what sighted user is going to want to hear vo blasting through a
speaker if you’re playing media? to me this is not a bug, but a welcome change.

This is truly funny. I believe it was a year or so ago that many blind users of
VO demanded that VO stay with the iPhone and music and other media play through
a Bluetooth speaker. This made a lot of sense because it allowed a user to
adjust volume and other settings, select a play list or source, etc, from their
phone without blasting VO through the speaker. This makes the most sense to me.
