How do you guys manage to get such good results? I have tried scanning
barcodes on cans, packages etc but can't detect anything. What's the
trick to get the code in camera focus?

I and my wife have been using it for years.
The iPhone app works great and we get a result within two or three
seconds.
We have our shopper put clear tape right over the bar code on the cans
and boxes at the time of purchase right in the store.
The tape is thick enough to feel and makes it much easier to know
where to scan.
The app makes a quiet beeping sound when it sees part of the bar code
which gives enough feedback to allow us to adjust the camera to pick
up the entire bar code.
As the data base of bar codes is on the internet, it is constantly
being updated automatically.
