Sounds like it's not finding them in the vpasswd file for the domain. I
would check the perms/ownership/mtime of the vpasswd file and
vpasswd.cdb file. Make sure that the user is being added to vpasswd and
that the modification time of the vpasswd.cdb indicates that it was
updated at the same time.
