Yep. AWL averaging from a _really_ high-scoring message sometime in the past is the most likely cause.

Clear that user's whitelist entry and the problem should go away. As was posted earlier, run:

 spamassassin --remove-addr-from-whitelist=problemacco...@clientdomain.com
