I had that problem on a shared FreeBSD server sometimes when I got a SARE update and the restart of spamd didn't work right. Found I had to stop spamd, sleep for a few seconds, and then start it. If I did a plain restart, the start part didn't work because some of the children were still alive. There is doubtless a more elegant solution, but that worked.
