Not really, if you aren't able to eliminated them without looking at
the image your choises are pretty limited: live with them or run
FuzzyOCR.
You might try running it over the most egregious accounts and see how
much of a hit it really imposes.
By the way, we use RBL to get rid of 80% spam at SMTP time, and
spamd is
run only on the rest of the mail that is coming through.
Have you looked into greylisting?
