Sure.  That'd be pretty easy, although (and I may be wrong depending on
the milter) you should have at least 1 received header all the time -- if
not, you're going to limit SpamAssassin anyway since it wants to do tests
based on who connected to you, which comes in via the received header.

Otherwise, it should be pretty simple to write a plugin for it.  There's
already an accessdb plugin which you could use as a base to do what you want.
